This ER diagram has been designed with bottom up approach. In Bottom up approach, you first design whole database schema, define hierarchy and relations between entities and when everything is finalized, you draw the diagram.

Both are weak entities as they don’t have primary key. We can convert treatment to strong entity by creating treatment ID as its primary key.

How to make ERD in case of multiple departments in Hospital?
If they have common features, single ERD will work for all the departments. If not, you can use the concept of generalisation and aggregation while drawing ERD.
